Health Information and why we collect it

  • A health service that is going to be provided to you

  • A health service you have asked to be provided to you

  • Some personal information for organ donation

Maria Andonopoulos Dietology collects health information from clients when it is relevant to the service provided, i.e. we need to know health information about you in order to provide you with nutrition advice.

However, in some circumstances we may be provided with information by third parties, such as a communication from your doctor or other healthcare professional. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.

We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect.

When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. However, most of the Personal Information is or will be stored in client files which will be kept by us for a minimum of 7 years.
